Medical Disclaimer Forms
PPARC have initiated a procedure whereby visitors to the JAC facilities on Hawaii will no longer be required
to provide a medical certificate, but will be warned of the potential dangers of working at altitude and be
required to sign a disclaimer. Disclaimers will be issued to groups by the PATT Secretariat following PATT
allocations. Signed disclaimers need to be returned to the PATT Secretariat for every person visiting the
telescopes, not just principal applicants.
Only one disclaimer per person will be required and it will remain valid for all approved programmes. It is
intended to renew these disclaimers every four years. Failure to provide a signed disclaimer will mean that
individuals will be denied access to the telescope(s). The form is also available on the JCMT homepage of the
World Wide Web.
