GENERAL STAFF NOTICE
PROCEDURES FOR EYE EXAM RELATED TO
VDT USE
As noted in the JAC Safety Manual,
Chapter
4, Section F5e, Video Display Terminal (VDT) Operators (i.e. defined as
those using display terminals for 3 or more hours per day, or 20 or
more
hours per week) are encouraged to take an annual Eye Test to
determine
if there is a requirement for eye glasses designed specifically for VDT
use. The JAC will meet any bona fide costs related to this test
that
are not covered by staff members’ health insurance.
In addition to the above, the JAC has
agreed
to reimburse the cost of eyeglasses (basic appliances) that have been
prescribed
specifically for VDT use. An estimate of the cost of any
prescribed
eyeglasses, for which reimbursements will be sought from the JAC, must
be submitted for prior approval to
the Human Resources Manager.
In order to qualify for the above
benefits,
employees must complete a video display terminal (VDT)
questionnaire.
This questionnaire is to be reviewed by your Vision Specialist
(Optician),
to help determine any effect VDT use may have on your
vision.
The Vision Specialist is then requested to note the results of the
examination
on the letter provided by the JAC. Copies of the
Questionnaire
and letter are available from the personnel section.
Staff may submit a claim for
reimbursement
of costs incurred, attaching the following:
1) Completed VDT Questionnaire
2) Letter
noting results of Vision
Specialist's Examination
3) Original Invoice for charges
