JAC Safety Manual - Manmods 21 Here is the twentieth round of modifications made to the JAC Safety Manual as per the JAC Safety Committee Meeting of 08-December-04.  Note: Modifications to the front and back covers will appear in the hard copy versions only.

      K.3. ciii. below.

  1. Before taking a JAC vehicle to Mauna Kea, take it to the designated service station and fill it with gasoline.
    • JAC currently uses the 76 Service Station, located at 370 Kaumana Drive.
    • Charge the gasoline to our credit card which is kept there and leave the receipt with the attendant.
    • When to fill-up gasoline:
      • 4-wheel drive vehicles: Always fill-up on the way to the summit because a schedule change may require that vehicle to remain at the summit.
      • 2-wheel drive vehicles: It is recommended that these vehicles be filled as well. This must be done if there is less than 1/4 tank of fuel.
As a courtesy to the next user, you should also If less than 1/4 tank gas remains then it
                        is recommended to 
fill up the tank upon returning to Hilo.

10 C2b.ix - 1st sentence: change to read:
     Under normal weather conditions the doors or doorsdoor shutters and roof may be opened by 
      any qualified person. 
10 C3 DEFINITION - 2nd sentence - change to read: 
      In the JAC system there will never be bothmore than one WL / TSS in a facility other than during 
      a change of responsibility (i.e. shift change).
10 C3 PURPOSE - 5th sentence - reorder to read: 
   As far as the summit facilities are concerned, the TSS/WL has been designated for ensuring, as far
    as is reasonably possible, the safety of all persons within it,  as well as responsible for 
    coordinating the safety of the facility,

10 D7b.ii - 1st sentence - change: and to but
When the roof is free from ice or snow, and the wind is below 50 mph, staff may go onto the roof andbut fall protection (full body harness and adjustable lifeline attached to roof panels) must be used.

On the upper and lower carousel roof panels use the adjustable lifelines and set them to prevent you from coming near the edge of the roof.
10 D8.h - delete last sentence:
When finished, put the fall protection equipment tidily back onto the board - the next crew will not want to spend 30 minutes untangling your mess!

In the event an employee falls, or some other related, serious incident occurs, (e.g.. near miss) the JAC Accident Investigation Committee shall investigate the circumstances of the fall or other incident to determine if the fall protection plan needs to be changed and shall implement those changes to prevent similar types of fall or incidents.

Electrical Safety Policy
MKD was actioned to ask TCC/EGS to review Chapter 12 assure that it  is up to date.

Follows the JAC's electrical safety policies and procedures and instructions of responsible supervisor.

12 E7. Table 1  - note definition of  "adder": 

* Adder is defined as: One that adds, especially a computational device that performs arithmetic addition. With regard to provisions for inadvertent movement, the minimum approach distances (working distances) must include an "adder" to compensate for the inadvertent movement of the worker relative to an energized part or the movement of the part relative to the worker.

Mauna Kea Vehicle 
Procedures Checklist
Driving/Vehicle Procedures - add following bullet:
  • Determine if vehicle is equipped with Antilock Brake System (ABS).
    • Check the instrument panel for an ABS indicator light after turning on the ignition. For more information, read the owner's manual located in the glove compartment of the vehicle.
